Bond Applications

These applications collect information used to complete or populate the corresponding bond or surety documents. The applications are not themselves approvals.

Bid Bond Application

Collects project details, bid amount, obligee information, solicitation number, bid date, bond percentage, and required bond form.

Performance Bond Application

Collects contract amount, scope of work, project schedule, owner/obligee information, contractor experience, and prior project references.

Payment Bond Application

Collects contract amount, payment bond amount, project owner, prime contractor or subcontractor information, scope of work, and contract documents.

Individual Surety / Asset Worksheet

Collects individual surety information, pledged asset details, estimated values, custodian information, and SF 28 requirements.

Approval / Decline Notice Template

Template for communicating approval, decline, or additional information requests to applicants.

Federal / GSA Forms

Federal forms should be completed according to the solicitation, contract documents, FAR requirements, agency instructions, and contracting officer direction.

SF 24 — Bid Bond

Standard federal bid bond form required for federal bid submissions.

SF 25 — Performance Bond

Standard federal performance bond form for federal contracts.

SF 25A — Payment Bond

Standard federal payment bond form for federal contracts.

SF 25B — Continuation Sheet

Continuation sheet for use with SF 25 and SF 25A.

SF 28 — Affidavit of Individual Surety

Required affidavit for individual surety submissions under FAR 28.203.

AIA Bond Forms

Where an AIA form is requested, FSA can collect the information needed to complete a licensed AIA bond form workflow. AIA forms should be obtained and completed through authorized AIA document sources or other properly licensed channels.

AIA bond forms are copyrighted documents. Do not reproduce AIA forms without the required license. Contact FSA for information on licensed AIA document workflows.
